Register with the surgery

Please Note: Currently our practice list is closed.  This means that presently we are only accepting registration requests for new babies from existing families registered with the surgery.  Parents of a new born who have been given a yellow infant registration form should still complete and submit the form to the practice.  

 

When our list re-opens, if you would like to register with the surgery our administrative team will be happy to guide you through the process. 

Call to collect the appropriate registration form or download below.  Photographic ID and one item which must contain your address are needed for patients registering from outside the UK. 

For patients registering with an HSC/R1 form, a copy of passport along with one of the following is required.  A copy of your Home Office document or receipt of application, a copy of your work permit or a copy of entry visa or dependants visa.

Those from NI/UK need an up to date medical card and an HS200 form.

It would be helpful if the patient could provide a list of current medication, immunisations (babies red book), allergies and any other relevant information.

In order to be able to register with this surgery you must be resident within the 3.5 mile radius practice area.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days.  After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to 3 months.  This will allow you to be on the local practice list and still remain a patient of your permanent GP.  After 3 months you will have to re-register as a temporary patient or permanently register with that practice.

Students home from University can also be registered temporarily if medical treatment is needed for up to 3 months.

To register as a temporary patient simply contact the local practice you wish to use.  Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.  You cannot register as a temporary patient at a practice in the town or are where you are already registered.
